This worked for a few weeks, once I learned how to do it.
The log-in URL is https://www.siriusxm.com/player/#view=login , and that is what I have in both the Keepass Title and URL. (It was learning about the Title entry that go this to work.) The entries are correct, as if I copy them to the clipboard, and then into a browser, the SiriusXM log-in page appears.
Ctrl, Alt+A, the default Global Auto-type is still set to that key combination.
Unlike last Saturday, for example, once I place the mouse cursor in the User Name Box and press Ctrl, Alt+A, today, in both Firefox 222.0 and IE9, nothing happens. Last Saturday, and at least a few Saturdays before that, Ctrl,Alt+A placed my User ID and password in their respective boxes and I could log on.
What magic incantation do I need to chant to restore this KeeyPass entry to viability?

Your KeePass Entry Title is incorrect. It should be: SiriusXM - Login
The KeePass default settings match the KeePass entry's Title with the Window Title (i.e. the text on the browser tab of the active webpage). It does NOT match the entry's Title with the URL in the browser Address Bar.
Note (this may not be relevant to your situation): If you happen to be using a browser add-on that posts the URL to the browser's Window Title, the SiriusXM page is able to override the add-on.

Thanks,that fixed the problem. Given that my previous title worked, and assuming that I did not change what that title was after last Saturday, I shall assume that the SiriusXM page title changed, and that I did not notice this.
I'm not in the habit of paying any attention to the page title shown in a Firefox tab, as that is not overly interesting; had I done so, I might have noticed the discrepancy between what I had as the title in KeePass, and what KeePass actually needed for the log-in to work. Of course, even had I looked at the tab, I still might not have noticed....
Add to that an anomaly which arose when I got your correction to work, one I've seen once in a while, where after entering my UserID and Password with KeeyPass, it gets rejected by SiriusXM, only to get accepted if I just try again without reentering the details as posted by KeePass.
In conclusion, again, thanks for the quick correction.

That sounds a little like KeePass is typing too fast. Some alternate auto-type sequences that might work (see Auto-Type keystokes for more information):
Add a delay (e.g. 500 msec, notice the absence of "=" in the placeholder) between typing the password and pressing enter:
{USERNAME}{TAB}{PASSWORD}{DELAY 500}{ENTER}
Add a delay between every keystroke (e.g. 10 msec, notice the presence of "=" in the placeholder):
{USERNAME}{TAB}{PASSWORD}{ENTER}{DELAY=10}
Change the sequence to press the login button rather than press enter (notice that the spacebar is used to press the login button):
{USERNAME}{TAB}{PASSWORD}{TAB}{TAB} {DELAY=5}

I shall try your helpful suggestions, but it may be analogous to trying to prove a negative: I don't log on to SiriusXM very often, so would not be testing any of the variables frequently enough to more or less know if the little problem has been solved by using one of them.
Saturday morning is the only "required" log-on, for the four-hour Baroque and Beyond program (and I missed the first hour or so yesterday, as I was out running, but logged on for my wife, who did hear much of that time span); other log-ons are just done if I happen to want to listen to some classical music on-line, with the exception of their wonderful Christmas music program which lasts for several days and which we like to hear
