In KeePass 2.X on Ubuntu Linux/Mono, I have to select the desired account twice...once to go to the desired web page/login screen, and again before I click on AutoType.

In KeePass 2.X on Windows 7, I only have to select the desired account once. That is, after selecting the desired account, going to the web page login screen, placing the blinking cursor in the username field and clicking on the padlock icon to return to KeePass, the desired account is still selected. So, all I have to do is click on AutoType.

How should this be reported?

You've just reported it.
It will be a mono bug / weirdness. You could try Global Auto-Type instead of returning to KeePass.

  I'm trying to set up Global Auto-Type on my 32-bit Ubuntu 12.04/Mono laptop and its not working. I think I need a newer version of xdotool than is available on the Ubuntu software center. On the xdotool website, I can only find the 64-bit version of the xdotool. Is there a 32-bit version available?

You have a later version than that required so it should work.

Global auto-type works when logging in to Amazon.com. It does not work when logging in to some sites, e.g. SourceForge.net.

Suggestions?

Are you sure there is a matching KeePass entry? The entry should either have a title of "Sourceforge" or a Target Window of "sourceforge*".

It works if I use the autotype feature 'Use custom sequences for specific windows'. I think my issue is solved. Thanks for the good coaching!!

KeePass uses the entry title to match the window title. e.g. on my machine this Sourceforge page has the title "KeePass / Discussion / Open Discussion:Selecting the Desired Account: Once on Windows; Twice on Ubuntu Linux/Mono - Mozilla Firefox".
To match my KeePass entry could have the title "Sourceforge", "Discussion" or "Selecting the desired account" - KeePass does not have to match the entire string. If you add the target manually you either need to match the entire string or use a wild card(s), e.g. "desired account".
