add authentication with certificate (alone or in smartcard/usb token/...)
Depending on timeframe/cost and so on, it could be sponsored by a big european firm.

However, you can write it as a plugin.
KeePass 1.10 will have a key provider API, which makes it easy for plugin developers to offer new key methods. You can find a documentation here: http://keepass.info/help/v1_dev/plg_keyprov.html
As KeePass 1.10 is not released yet (it probably will in the first week of 2008), you of course can't compile the sample code yet. If you find someone willing to develop certificate / smart card support, please let me know and I'll send you the source code of the current development snapshot immediately.
