Computing evironments are rarely risk free. For each particular environment where you use your database you must decide whether leaving your database open and usb drive accessible is an acceptable risk. It is up to you to secure your devices to reduce those risks to acceptable levels.
If you leave your database open, or you leave your usb drive containing your key file attached to your device then it may be possible for malware, or someone having physical access to your device, to steal the data.

KeePass provides some protection of open databases against generic (most) malware, but can not protect against malware that specifically targets open KeePass databases.
In the presence of active malware, and dependent on the nature of the malware:
Your data is safe if the database is closed, and the Master Key is strong and inaccessible.
Your data might be vulnerable if the database is closed, and the Master Key is weak or accessible (e.g. on an attached USB drive).
Your data might be vulnerable if the database is open, regardless of the Master Key strength or accessibility.
