Yes. Transfer your database file (probably ending in .kbdx) to your mobile device in any way you like. Then install an appropriate port of KeePass for your device. Several are listed on the KeePass download page near the bottom.
If you want to automatically update with changes from your PC and/or sync changes from your mobile device as you edit entries, the best way is usually to simply put your database file in a cloud storage system such as Dropbox or Google Drive. On your PC, you can install one of the many "backup and synchronization" KeePass plugins for your cloud storage of choice. Alternatively, do it manually using the sync folder for your cloud storage client and setting up a trigger to sync a local copy with a cloud storage file. I find the plugin method more straightforward. On your mobile device, many of the ported apps can open directly from cloud storage.
