Hi, I have installed the customized request form extension but upon upgrading to itop 3.0, we are now encountering errors when submitting a Ticket.

I have attached the error that we are encountering.

Hello,
This is indeed an extension version compatible with iTop 3.0, so this isn't what is causing the issue.
Can you check your log/error.log, and if nothing found the ajax response in your browser devtools ?
