Looks like somebody on your end added that.  I'd saved my old Privoxy
3.0.3 directory; just checked and there's no "disable-insert" string
to be found in any of the files.

Check the {{alias}} section of your old default.action & user.action
files.  It'll look something like the crunch-all-cookies alias:
  +crunch-all-cookies = +crunch-incoming-cookies +crunch-outgoing-cookies
