        Driver          "nvidia"
        VendorName  "Videocard vendor"
        Option      "TwinView" "true"
        Option      "TwinViewOrientation" "LeftOf"

This puts the second monitor on the left.

In icewm I can tell it which side to put the menu, and so on.
