#594 Better create table (columns area)

I think the Create Table > Columns a bit disorganized and limited, and in
future to give more work.
The suggestion is: remove structure of current columns area and make a
listing with ADD, DELETE and EDIT an column.
On click in ADD will open the ADD FIELD DIALOG.
Motive: easy to work (example: if you edit anything in ADD FIELD DIALOG,
don't need update too the CREATE TABLE DIALOG), some bugs too exists in
current system (try by example, in CREATE TABLE DIALOG, add a new field
and set how "Varchar", the Autoincrement option is enabled).
