I'm seriously doubtful it is all this simple.  For one thing, this
explanation completely fails to explain why the changes from 4.0.2 to 4.1
made the problem go away.

Not necessarily.  It seems to depend on the way used to do the conversion.

And there's still not the first step towards an explanation of the Moir'e
pattern in the colorbox in sight.
