#43 Fix for the "Combined" node style in 0.8 RC2

The "standardnodestyle=combined" setting in Freemind
0.8rc2 has no effect. It appears (from looking at the
source) that this nodestyle is intended to preserve the
old freemind 0.7x node style (which I prefer) where
folded nodes are bubbles, and non-folded nodes are forks.

The attached patch fixes the problem.
