There was an episode of "Air Emergency" on TV the other day about an airliner 
that ran out of fuel over the Atlantic on a flight from New York to London 
because of a fuel leak.  They ended up gliding 80 miles to a dead stick  
landing on the Azores.  The plane landed with minor damage because they had 
too much speed/altitude on approach and put it down a little hard and fast.  
There were no injuries.  It turns out that the wrong fuel pump had been 
installed when an engine was replaced and this caused the fuel line to chaff 
through which resulted in a huge fuel leak.  The pilots did not notice the 
leak until minutes before the engines stopped.
