[Cctools-cchost] checkin: html in ban message and other tweaks

Almost done with the sample browser.

Also added ability to use HTML in the ban/moderated message.

This includes a scary change to forms and templates so if there's
anybody out there grabbing daily builds please bang extra hard on
forms, especially where html is involved.
