Are we doing something wrong? We don't use Costello, because at the beginning there was several problems, so I'm not familiar with Costello-problems. I only can guess what's going wrong. I suppose that the Dialogs "Delete User *" are childrens from "Delete User", if Costello named different dialogs with the names "Delete User*". The other idea is, that Costello named the dialog "Delete User" with different names. In this case make sure that the Dialog is really disposed. Hope that this answer is a little helpful for you.
