It is strange to have vaccinations in the History screen with N/A, normal, abnormal.
You give a vaccination or you do not give a vaccination.
TBC, HIV, and other laboratory test can be abnormal or normal.
So only if the medication is an issue. Allergy or other different problem you will add it through ISSUE list? What happens with the running medications under menu choice Rx? More than one medications could give interference and might give contradictoy effects ;-((
BTW: I think Vaccinations should be found under immumization menu .

Are you talking about the Risk Factors screen? That's not what this MU is about. It would be better to open a new discussion thread for it.
Your medication question seems to be related to drug decision support. That is already handled by another team, who is actively working on it now.
