Is this specific to the SquidGuard module? If so, then it is probably a bug in interracting with the Berkeley DB. I haven't spent much time on bug-hunting in that module yet (too many irons in the fire), but I've been led to understand by a few folks that the DB handling code in the module is sluggish (which probably indicates locking issues, or similar--it is easy to make mistakes like that when dealing with a lockable db). If you can reproduce it reliably forward over a bug report to me or Tim. (Tim wrote it for us, but I think he is pretty busy with other projects right now, so I'll be fixing some of the issues at some point in the near future myself.)
