I know others have also asked about this, on this list or elsewhere--I believe Jeff Hobbs has noted that the Mac icns icon format is a bit opaque, and that writing a Tk extension to parse icns files is difficult. I believe that Tk on Windows can match icon files to file type via the ico extension (or TkImage) and parsing the registry. If I'm reading the Carbon docs correctly, it isn't necessary to use so many steps on OS X: the Carbon frameworks should take care of most of this.
