mv [Path to patch]/smallcal-i18n.diff.gz [Path to SquirrelMail]/plugins
cd [Path to SquirrelMail]/plugins
gzip -d smallcal-i18n.diff.gz
patch -p0 < smallcal-i18n.diff

Not tricky at all when you know it.
