Hey Alyson, you always need both. To start out, just use the Special:CreateClass page. I normally call the template, the form and the category the same, e.g. "Contract" in your case. Then just fill in your properties and give them the right types, e.g. "has vendor" (type Page), "has start date" (Date) and "has end date" (Date). If you want, fill in some field names to appear in the form, e.g. "Vendor:", "Start date:" and "End date:". (If you want to query the description you would also need a property "has description" (Text), but instead of a property you could also use the free text for that.) Hit the Create button and you will get some properties, a Category:Contract, a Form:Contract and a Template:Contract. The latter two you might want to tune to your needs. Have fun, Stephan.
