The current interfaces for writing extension instructions are very low-level: I think it almost certainly would be possible to use them on .NET, but it would require some pretty deep understanding of the Saxon internals as well as the IKVMC technology. In principle I would like to provide a higher-level, more abstract interface for extension instructions on the Java platform, which would then also be easier to use on .NET. However, extension instructions haven't proved vastly popular and are not widely used, so it's not really a high priority at present.
