Are you using any external authentication methods? I recently had a bug on the Debian package where the user was presented with a completely blank page when trying to login because they had configured IMAP authentication but neglected to install the IMAP extensions for PHP4. I filed a bug on PHPwiki at: http://sourceforge.net/tracker/index.php?func=detail&aid=1334024&group_id=6121&atid=106121 to ask for improved error handling in this situation.
