Fixed a few bugs.

* include appropriate quotation marks in entity declarations

* print correct output filenames in external entity declaration file

* print correct location of driver files in internal subset of main output
file

* create directories where needed when writing output files of any sort

* output all files under output directory, even those with absolute paths
(so when converting the external entity file /usr/local/text/entity.sgm,
write ./usr/local/text/entity.xml)

* don't redefine predefined entities (such as amp and lt)

* clarify doc on preserve-external command line switches

Thanks to the SGML->XML migration TEI working group for catching these
problems and discussing the appropriate fixes. (Guys, note that this
represents a change in the OpenJade CVS archives and not in the available
tarball.)
