A hyperlink in a "Table of Contents" in OpenXML needs a bookmark element as target.

Such a hyperlink in ODF doesn't have a real XML element as target. The target of an ODF hyperlink is a formatted string that contains the heading numbering and the text of the target heading.

The converter needs to create a bookmark next to the heading element when converting a ODF "Table of Contents" to OpenXML.
For that reason the converter tries to find the matching heading by determining it's outline level and text in the document.

This may fail if there are more than one headings with the same outline level and text. In this case such a hyperlink will no longer point to the correct heading.
