I have found that if the setup.exe is larger than 250 MB it does not inject the icon into setup.exe. This is true even if I do not use my own - the default icons are not coming through either. When I remove some of the files that are included in my setup program, the setup icon show up again... It won't query icons from executables larger than a certain limit.
