I sometimes find mail in my drafts folder that I thought I had sent. I think what's causing this is typing C-c C-c to send the message, then powering down the system before the mail has actually been delivered to my ISP.
It would be nice to get some sort of visual indication that outgoing mail is still being processed.
When I brought this up on the users mailing list, Bill wrote

asynchronous tasks. So we could, in theory, monitor
that asynchronous task asynchronously. Where we add
the "Mail" text to the mode line, we could also add
"Send" text.
