Two diacritics using combining characters doesn’t work

For IPA, I have to put the tilde above characters (including ɛ and ɔ), and above the tilde I have to put acut, grave, circumflex and macron. As there aren't Unicode code points for this, I've used the combining tilde and the combining acut, combining grave, combining circumflex and combining macron. Result: both diacritics are paintet at the same position. Expected result: one of the diacritics is above the other one.
Doesn't work on ASCII characters either. Current and expected result (using SIL fonts) attached. For some reason stacking seems to work in some cases ('V' + macron + breve).
