Yes, the refinement patters are devised such that the child side == parent side. Among other things, this allows us to make assumptions in the BoundaryInfo class. As for pyramids, they used to refine uniformly in libMesh, but when re-factoring the refinement scheme I never implemented the embedding matrix. I figure I'll wait for the RefinementPattern implementation, since there seems to be no big rush for it right now.
