I'm guessing the problem is that sourceforge.net is the last line in the block list.

Some simple changes would make it a lot better. For example
/ad.js
blocks "http://foo.example.com/ad.js" but not "http://foo.example.com/bar/ad.js" in Privoxy. If you change the filter to
/(.*/)?ad\.js
it will block both of the above example URLs.

& typing URLs into http://p.p/show-url-info is ok, but please consider creating a list amenable to automated testing of your patterns. See
