#389 Add User Comment Field

I often find I can't remember why I blocked a particular URL, or unblocked a URL, etc. It would be useful to have a User Comment field next to each URL.

An example: a URL is unblocked. Months later I notice I want to block it. I go to edit the actions file and notice the URL is specifically unblocked, but I can't remember why I unblocked it. I must have done it for a reason, but why? A comment would allow me to decide whether to keep it unblocked, or change it to blocked.
