What about in webapps/geoserver/WEB-INF/web.xml?  I think that's the 
location we were referring to, I'm not sure if webdefault.xml gets 
picked up...

Or geoserver/server/geoserver/WEB-INF/web.xml if you've got a binary install

To be extra sure get it completely off and back on.  web.xml may only

hrm.  This is a bit weird, it seems like the proxy should be able to 
handle this...  But I don't know that much about proxies...

Is GeoServer in tomcat or are you running the binary version?
