One technique for creating the outdoor terrains you mention that might interest you is using a heightfield generated terrain and a texture synthesizer in conjunction with an infrastructure map. The synthesizer would generate grass/dirt/rock/whatever areas from the information in the heightfield. The infrastructure would 'override' the synthesizer textures and blend things like roads, farming fields, and other human (or natural) created infrastructures into the world. If you work of a 256 grayscale imags for the heightfield and a 256 color image for the infrastructure map, these could be fairly easy created in simple 2D editors like PaintShopPro.
