For a quick fix, replace your
EXIST_HOME/tools/XFormsFilter/resources/xslt/xhtml-form-controls.xsl
with the one from here -
http://exist.svn.sourceforge.net/viewvc/*checkout*/exist/trunk/eXist/tools/XFormsFilter/resources/xslt/xhtml-form-controls.xsl
