When emulating an EGA, some of the BIOS video modes have incorrect horizontal and/or vertical totals. Also, DOSBox does not switch to the 16 MHz dot clock in 350-line modes, resulting in an incorrect 52 Hz refresh rate.

This patch fixes the incorrect horizontal/vertical totals, and switches to the 16 MHz clock when appropriate.
