#36 TDG: Patch for chapter 4 of TDG

* Use CDATA sections in screen and programlisting
blocks containing sample
SGML and XML code.
* Put quotes around attribute values in one sample
block. Some of the
attributes are attached to <xref> tags; so far as I
know, there is no way
to represent an <xref> that is legal in both DocBook
SGML and XML, so I
left them SGML-compliant (without a closing /).
* Update the Emacs comments at the end of the document.

I expect to make more changes to chapter 4 later; I'll
attach
a revised patch to this report (assuming it hasn't been
applied).

Here is a revised patch to wrap more programlistings in
CDATA and
make further markup changes. The original patch should be
discarded.

Revised changelog:

* Replace “/” pairs with <quote> tags.
* Eliminate “/” entities used following an
<xref>; this
may be an outdated stylesheet workaround.
* Replace a quoted reference to a section with an <xref>
(Scheme).
