I strongly suggest you re-evaluate whether you actually *need* to read 
the file yourself.  cscope is expressly designed to avoid that need, and 
so far it's been doing quite well on that front.  None of the existing 
programs using cscope as a back-end read cscope.out themselves.  They 
all use cscope's line mode (the -l option) in a subprocess.

No.  Because it's not really needed for anything, nobody bothered to 
write such a document.
