#871 Custom annotations removed / missing from template and calls to XSLT

When you add custom annotation tags to an item they save to the CAM template and the CXF.

However - when you close the template and reopen it - then they are not loaded - they are ignored.

Similarly - when you put in Added and Changed annotations - and then run the MindMap view - those color codings don't work - because what gets passed to the XSLT does have those annotations in it.

This is also effecting the Tabular Documentation as well. Only the Definition annotations appear to be sent to the XSLT.
