Upgrade or reinstall latest 3dfx drivers. Remove opengl32.dll from quake dir and make sure you use opengl icd. It has helped for all the V3/V2 users I've talked with so far. Do not use minigl in quake folder.
Use latest drivers from 3dfx, not wickedgl or others, if you have any problems with quakeforge. 3rd party Windows drivers are known for incomplete or incompatible implementations of OpenGL.
