This new update fixed the issue, thanks! :D
I am using Windows 10 with scale set to 100% in Display Settings. I tried setting compatibility mode to Windows 7 and that did not change anything. I tried changing all of the settings in "Change high DPI settings" and those did not do anything either.
If you select a game from DxWnd and right click and "Modify" it you'll get to the "Main" page tab. From there if you look in the bottom left you can see that the position box is cut off when trying to input values, making it hard to see.

Sorry for the late reply, I didn't get an email notification like I usually do :c Anyway, I appreciate your help. I was able to get the game to run with dgVoodoo 2 so I can run it at higher resolutions :D
I was wondering, could you help me run the game at 2560x1440 by chance? I attached the log file of the game crashing at 2560x1440. Hopefully this can help us find the issue.

I just came across something interesting. If I open the D3DConfigUtility and set the game to 800x600 then launch the game from DXWnd with "Initial Resolution" set to 800x600 the game will actually run in windowed mode. Now the trick is getting the game to run at a higher resolution...
