Tested on Windows 8.1 - FileOptimizer 9.80 Setting Log level: Information, will show all commands in the log. The log shows "cpdf.exe -squeeze" I tried that book and it worked with Mupdf, Ghostscript, and Cpdf enabled. Disabling Ghostscript gave broken output identical to yours. For this file (or any with lossy images), Ghostscript should be disabled because of image quality issues - https://sourceforge.net/p/nikkhokkho/tickets/5/#c441. Use Mupdf for now.
