Hello! I spent a couple of hours to set up a new Yubikey FIPS (similar to Yubikey 4) with KeePass 2.39.1 and OtpKeyProv 2.6 but I cant open the database only with recovery mode. Followed the instructions, and the topics from help. https://www.yubico.com/why-yubico/for-individuals/password-managers/keepass/ I configured the second slot, it generates the OTP codes, but the error every time is, failed to create OTP key. I use English (US) keyboard, the yubikey generates numbers. Where I missed something?...

Meantime I experimented that I have to specify minim a 1 in the Look-ahead count to access to the database, thx.
