OG Forms is a browser-based form builder for branded forms, surveys, and quizzes. Users can create forms from a prompt and refine them with AI chat, then customize themes, fonts, colors, layouts, and conditional logic. The builder includes more than 30 question types, quiz scoring, calculated fields, answer piping, file uploads, signatures, and hidden fields. Responses can be reviewed with AI summaries, exported as CSV or JSON, restored from an answer log, synced to Google Sheets, or sent to other services through webhooks. Shared workspaces support team collaboration, and spam controls help manage public submissions. OG Forms is designed for individuals, small businesses, educators, and teams collecting client intake, event registrations, feedback, applications, and similar information. Its free plan includes unlimited forms, responses, and teammates; paid Pro options add branding controls and a custom domain. Owners can transfer workspace ownership or export a full workspace.

NoteForms is a no‑code form builder designed specifically for Notion that lets you create and deploy fully customizable multi‑page forms and surveys that feed directly into your Notion databases. You can tailor every detail, colors, themes, logos, covers, and even custom domains, while leveraging advanced form logic like conditional branching, relation field support for selecting linked database entries, hidden and people fields, and file uploads or digital signatures. Built‑in notifications via email, Slack, and Discord ensure you’re alerted instantly on each submission, and customizable confirmation emails provide respondents with real‑time feedback. Editable submissions, analytics dashboards, and dark‑mode support simplify form management, while white‑labeling and custom JavaScript/CSS options let you remove all NoteForms branding and embed forms anywhere as standalone pages or popups. With unlimited forms, unlimited responses, and seamless Notion integration.
