This is another bug fix release that also includes a performance enhancement that was going to be added in 0.3.0.

What's changed in this release:
* The basic generators now use sample tables, meaning they perform at higher speeds.
* The fourth internal channel (from the user's perspective) is no longer silent.
* Better detection for platforms on which the CPU meter will work at compile-time.
* The PortAudio audio backend now doesn't crash the whole program if it can't initialize.
* PortAudio no longer initializes twice, making for less verbose program startups.
