About PATHOS-WEB Free

PATHOS-WEB is a software package which enables reliable recording, coding (SNOMED & ICD) and reporting of pathological data. Fast and accurate search capabilities are its core competency.

PATHOS-WEB is a 32 bit client-server application developed by the latest development tools (MS Visual Basic) running on any Microsoft Windows environment (XP/2000/ME/98/95 or NT). The database management system (DBMS) is MS Jet 4.0, yet any other ODBC or ADO compliant relational or Object Oriented DBMS may be used (Preferably MS SQL Server). This flexible feature allows seamless integration with Hospital Information Systems.

SNOMED Pathology Microglossary is included and fuly integrated with the application. There is a very efficient and easy-to-use automatic coding module which you will love it!

The Imaging Module allows to capture, compress and store associated with Pathologic Records. Basic image editing functions like zooming, panning, annotating and exporting are included. The imaging module uses the legendary Eastman-Kodak Microsoft Imaging Components.

Also for creating top quality standards based cytopathology reports, the Bethesda System 2001 module is included. It is possible to enter data by simply point & clicking. Edit checks and semantic checks are done on the fly. Finally an automatic report generator prepares the report text to be printed with other data elements.


GENERAL ISSUES

PATHOS-WEB is truly multilingual by its design. It currently supports English and Turkish versions, but any language can be added easily. Please ask for it and possibly help me!

Its user interface may look strange at first; but this simple design has been the result of 10+ years of experience in live clinical settings. I strongly believe that any software must be stupid simple and be learned in no more than 5 mins in order for doctors (and other healtcare staff) to accept using it...You may argue that this is unacceptable and that my thoughts are somewhat ancient/over conservative but...Hey I have been doing this since 1995 and number of users and ever increasing user satisfaction proves that it works! And keep in mind that I am also an MD that got into this business because I was not satisfied with what was given to us by industry...

The only 3rd party component used in the project is again another great Open Source Report Manager tool by Toni Martir. (R.M.ActiveX 2.4)

You will probably want to download the Report Designer component so that you can change the design of existing reports or even create new ones as you need. The designer you need is R.M.Designer 2.4. The details are below:

There are also two other components in the project as ActiveX Dll developed by me.
1) Imajlar.dll ==> The imaging component
2) Bethesda2001.dll ==> The component needed for Structured Data Entry and Automatic Report Generation for Cervical Smears conforming to the Bethesda System 2001. For further information look at URL: http://bethesda2001.cancer.gov/

LICENSING ISSUES WITH SNOMED Pathology Microglossary

Since the controlled vocabulary is not a free service, I am not allowed to give away more than 10% of the whole set. So the version included in the free Internet download is the cropped version. Please do get into contact with the licensing authority in your country.





SETUP ISSUES

The setup is created with the great Open Source NullSoft installer 2.11 using Modern GUI (MUI). I have tested it with Win98 English and WinXP Professional English platforms. In case a recent Microsoft Data Access Components (MDAC_TYP.EXE); such as version 2.8 is not installed the installer gives a warning. So you must simply go to the microsoft site, download and install an appropriate language version of it.





MAIN CONFIGURATION FILE: Ana.mdb

The general application options such as language, codepage, Server Share, and etc. and user preferences such as number of signing persons, default signatures, and etc. are store in an MS Access file in the Install directory: Ana.mdb

This file also contains localisation strings for the whole application for both English and Turkish

Basic configuration of the application can be performed from System Operations so you do not have to bug around with this sensitive file. But for details on the configuration please contact me.




DATABASE ISSUES

The application is installed with preconfigured basic Database Management System (DBMS) MS Jet 4.0 (MS Access 2003). In standalone configurations or very basic client-server environments with low data input you may be happy with this. The demo Access database comes with some 500+ dummy records for playing around with application. When you decide to start using for routine practice you may simply delete the existing dummy records (by using Access, open table B and delete all records)

But the DBMS of preference is MS SQL Server or the free desktop version Microsoft Data Engine (MSDE) or the latest MS SQL Server 2005 Express. Oracle version 8 and up is also supported but NOT RECOMMENDED!

In case you decide to install a more reliable and scalabe DBMS other that Access please contact me to get configuration scripts and other support.

The database model is as follows (At installdir/Data)

Genel.mdb: Main database to record all entries. Contains two related tables:
	B==>Just a flat table that holds everything with primary key BIYOPSINO
	Bethesda==>Related table (one-to-one relationship) to store structured data for Bethesda System 2001.

System.mdb: The supporting system information is stored. The tables are
	BOLUM==> List of departments (for drop-down list in main form)
	DOKTOR==> List of signing doctors and possibly transcriptionists
	FATURA==> List of patient financial types
	ISLEM==> List of Procedures (CPT or custom) with monetary entries
	M==> Standard templates to be used in Macroscopy/Microscopy/Comments
	SEKIL==> List of coded templates for Accession
	TARIFE==> Different price lists for procedure list
	TTB==> Turkish Medical Association coefficient for determining monetary figures
	YER==> List of coded templates for Source of specimen

Code.mdb: The SNOMED Pathology Microglossary (Cropped %10 version due to licensing issues)

Synonyms.mdb: For recording of custom synonyms and terms for your needs. The system learns as you start coding and then remembers in subsequent entries.

Exclude.mdb: List of terms to be excluded from automatic parsing and coding process. Can be customized as you start coding

Results.mdb: Database to store search results with criteria and also billings operations. Tables are

	ARAMA==> Table to store search results
	BETHESDA==> For Bethesda related SDE criteria
	ISLEM==> Table to store Billing related queries
	KRITER==> Search criteria
	RAPOR==> Temp. storage for printing of reports
