That is correct, because the item(s) in the Inventory is/are copy/copies of the Custom Item once they are created with a different life-cycle.
Some of the items in the Inventory are deployed onto the Layout (and removed from the toolbar).
It is a bit of a mess, and I also found an Abend bug playing about with it with the Description field.
You'll notice two buttons in the New Car Dialog. "Add" and "New". Add simply adds a Car using a Prototype. But New allows you to add either a Part (and Prototype) or just a Prototype (via a pop-up menu).
In the New Part window the "New" button allows you to define a New Prototype first...
Let's think about workflows
A) I am adding things to my layout -> Car Inventory Dialog. I either have a prototype and want to use it, or (New Car from Prototype) I have an existing Car I would like to clone, or (Clone Car) I have neither and just want to define a Car (New Car with no Prototype (yet)). If I delete a Car the Prototype is unaffected. If I unload or delete a prototype, the cars/inventory is unaffected
B) I am building a library of parts that others will use. I want to define a Prototypes with Cars that are tightly coupled so I can iterate the design -> Manage Prototypes (no dedicated Panel exists for this today) I want to define the Prototype but see how it will look as a Car, edit/re-edit it. If I delete a prototype or change it, I want the Cars to instantly change as well.
