Modified fixpoint scheduling to use a scheduling queue to avoid linear scan of completion frames. Updated adjust_levels to be more multi-thread friendly, I hope.

Fixed adjust_levels to be linear. This changes how leaders are found.

Fixed a bug in inline_builtin for putenv.

Moved the tnot call to answer_completed after the check for completion. Improved some printout for debugging logging (not enabled.)

Fix answer-completion to correctly handle already AC-ed subcomponents, so is not unnecessarily quadratic. Also use new abolish_table_pred without cps-check and table gc to avoid unnecessary quadratic behavior.

Allow abolish_table_pred_bi to take a parameter to turn off cps_checking and table gc.

parse.P - better comments/documentation for open_or_xpp_file.
tables.xwam - somebody forgot to commit

Another bug fix in answer completion, noted by Jan.

Added documentation for load_dynca, and callouts in places load_dync is mentioned. Make it easier for benchmarkers to find it.
