changed intern_term to throw errors instead of fail.

Add mode declarations, and some minor checks, to ensure that all syslib file pass groundedness mode checking.

Added new hook calls to support intercepting assert and retractall.

Added new hooks: xsb_intercept_assert_hook and xsb_intercept_retractall_hook, to allow asserts and retractalls to be replaced (mostly so e.g. assert(p(a)) can be turned into assert(p_int(a)).

Adding new files with inferred mode info.

turned on test of intern_term for large arities. It should now work.

Forgot to include changes to calling builtins in changelog.

First draft of changelog for new release, and minor changes to manual.

Minor formatting changes.
