Added documentation of load_dyn predicates to load into a specific module.

Added documentation for encoding directive. Updated other related documentatin.

Updated to handle new encoding directive and to start reading load_dyn files in utf-8.

Update to support new encoding directive, and reading of compiled files in utf-8. (Also fix old testing code in table_index.)

updated tests to accord with new :- encoding directive, and default of utf-8 for XSB source code (for compiling and load_dyning).

changed io_builtins back to the way it had been. I dont really agree but I dont want to fight.

Added warning when a hash-chain of length > 30000 is run in assert.
Warns of common performance error in XSB.

Updates to reflect that we're now number_chars-ing better.

Fixes to write floats in the standard format (which is changeable).

Took out very obsolete and very bad code for floats in term to codes.
It now just uses number_codes.
