changed the default on windows32 to __cdecl
For stdcall'ers, configure --with-stdcall
Crucial changes in export.h, config files, and various package NMakefiles.

Note: Win64 ignores _stdcall and always uses __cdecl.

base64_enc.c: casts to quite compiler.
biassert.c: handle 0-ary predicates for profiling.
builtin.c: minor cleanup around profiling (plus array overflow fix.)

Modified printout to include a predicate only if it has at least 0.1% of
the total time.

fixed a typo, which broke http PUT requests

Fixed a couple of bugs in the SCC analysis of max_scc_subgoals_header.

Now we postpone abstraction when we hit the maximum subgoal size in
the middle of a attributed variable, rather than aborting.
