XSB Src (SVN)-old

Pass at the table inspection predicates of Chapter 6. I intend to
fold Chapter 6.15.2 into the discussion of inspection predicates in
the Debugging chapter.

Added a test of memory tripwire suspension.

Caught more cases for max_memory tripwire (including heap expansion!)

Now invoking max_memory tripwire on tcpstack and compstack expansion.
Still need to do a couple more,

Fixed bug in initialize_state in cleaning up isa_ext and hasAttr_ext
facts when using the _mti indexing.

Added predicate_defined/1 to check whether a predicate has been given
a definition.

Added new dump_extensional version, which dumps a subset of an
ontology based on the class hierarchy. (Actually this is a poor man's
verson of components, that doesn't depend on the source, which we
never were able to manage correctly.)

Added casts to quiet my compiler. (Teri, you should check. I saw int
casts elsewhere for these flags, but int seem rather small to me for
memory sizes?)

Support for "suspend" action for max_memory tripwire.
