Added definitions for get_mod_for_psc, and get_mod_name.

Improved an error message in one cse when getc returns an error. (Previously
everything was treated as eof.)

Removed logging, and improved (I think) a comment.

Added new versin of c_string_to_p_charlist that includes length of
string, to allow construction of code lists with '0' bytes in them.

Change get_mod_for_psc to be a macro; so delete code here.

Added module name to error messages printing erroneoulsy declared
predicates for incremental tabling.

Fixed bug in garbled snprintf length. Also added many casts to quiet
the linux (gcc) compiler.

added table to incremental abolish set. (Table was erroneously missing.)
Also removed debugging logging.

Added documentation of timed_call_modify/1 and current_timed_call/2.
Also cleaned up some indexing statements.

Added a cast to quiet compiler.
